Here are the steps to becoming our patient. Please read through and follow the steps carefully.
1. Call our office
We highly recommend reading through our website to assess if we would be a good fit. Please read through our FAQs as well as our Insurances and Rates.
If you feel that we may be a good fit for you, give our office a call at (858) 354-1304 to schedule your first appointment. We will check if we accept your insurance, walk you through our first appointment scheduling process, and answer any questions you may have.
2. Fill out our intake and telepsychiatry forms
Please scroll down below to access the forms we require you to fill out prior to your first appointment. These forms can be filled out at our office before your appointment.
We require all the forms to be filled out. In addition to the forms, we also require images of the front and back of your Insurance Card, Driver’s License, and Credit Card to be on file.
3. Submit your forms and the front and back of your cards before your appointment
If you do not want to arrive early to your appointment to complete your forms, please email or fax us your completed forms and images of the front and back of your Insurance Card, Driver’s License, and Credit Card to be on file at least one days prior to your appointment.

6. Initial Medical Questionnaire
If you do not have access to a printer, we recommend using Adobe Reader (Windows Operating System) or Preview (Mac Operating System) to fill and sign. Further instructions can be found here.
If you do not have a physical scanner, we recommend downloading a mobile app like Adobe Scan or Microsoft Office Lens on your smartphone to scan your printed forms.
