Here are the steps to becoming our patient. Please read through and follow the steps carefully.
ATTENTION: Please aim to fill out and turn in your new patient forms right after you schedule your appointment.
We require completed new patient forms before seeing you as a new patient. Due to overwhelming new patient requests, we ask that you please turn in your forms at least 2 days prior to your appointment.
If you fail to turn in your form on time, we will be under the impression that you do not want your appointment, and we will cancel your appointment to accommodate patients with immediate needs.
1. Call our office
We highly recommend reading through our website to assess if we would be a good fit. Please read through our FAQs (https://clearinsightpsychiatry.com/faqs/) as well as our Insurances and Rates (https://clearinsightpsychiatry.com/insurances-and-rates/).
If you feel that we may be a good fit for you, give our office a call at (858) 354-1304 to schedule your first appointment. We will check if we accept your insurance, walk you through our first appointment scheduling process, and answer any questions you may have.
2. Fill out our intake and telepsychiatry forms
Please scroll down below to access the forms we require you to fill out prior to your first appointment. We require all the forms to be filled out. In addition to the forms, we also require images of the front and back of your Insurance Card, Driver’s License, and Credit Card to be on file.
3. Submit your forms and the front and back of your cards before your appointment
Please email or fax us your completed forms and images of the front and back of your Insurance Card, Driver’s License, and Credit Card to be on file at least two days prior to your appointment.
It is imperative that you turn these forms in as soon as possible and at the latest two days before your appointment. If we do not receive these documents two days prior to your appointment and do not hear from you, we will cancel your appointment to accommodate patients on our waiting list.

4. Expect a link or text prior to your appointment
When we set up your first appointment, we will ask you whether or not you would like to do a telemedicine appointment via smartphone or via computer. If you choose the smartphone option, we will send you a link to the appointment room just before your appointment time. If you choose the computer option, we will send you a link to the appointment room the day prior to your appointment.

Initial Visit Forms
If you do not have access to a printer, we recommend using Adobe Reader (Windows Operating System) or Preview (Mac Operating System) to fill and sign. Further instructions can be found here.
If you do not have a physical scanner, we recommend downloading a mobile app like Adobe Scan or Microsoft Office Lens on your smartphone to scan your printed forms.
If you have technological difficulty filling out and sending these forms or would like to fill out a paper version and do not have access to a printer, please give our office a call (858) 354-1304, so we can help!
When we return to in-office appointments, we will have these forms ready for you to fill out at the office. Currently, we will be closed for in-office appointments until further notice for your safety during COVID-19 pandemic.
